1. An Image Of Your HEALTH INSURANCE CARD

2. You’ll Need Information Including:

   1. Any Known Allergies;

   2. Any Medications Taken Regularly;

   3. Confirm The Athlete Has Had A Physical Exam Within The Last Year; And

   4. U.S. Ski And Snowboard Member Number/USASA Number, (If Applicable).
