2. An Image Of Your HEALTH INSURANCE CARD

3. A HEAD SHOT

4. You’ll Need Information Including:

   1. Any Known Allergies;

   2. Any Medications Taken Regularly;

   3. Confirm The Athlete Has Had A Physical Exam Within The Last Year; And

   4. US Ski And Snowboard Association (USSS) Number, If Applicable.
